We Bring Day Habilitation to You

Through our Community Habilitation program, Opengate staff members deliver personalized support to people in their homes or within the community. Community Habilitation-R is specifically designed for those living in certified settings and focuses on providing services exclusively in the community.

Opengate staff collaborate with the people we support and their families to create a tailored program that meets their unique needs. Program-related transportation is provided as needed. Activities are self-directed and aim to advance independence, community participation, and capacity building, all while prioritizing safety, well-being, and comfort.

Our goal-oriented support encompasses skill development in various areas, including daily living skills, travel, health and wellness, adult education, communication, socialization, leisure skills, money management, positive behavioral support, safety skills, self-advocacy, informed choice, community integration, and relationship building. Additionally, we offer personal care, healthcare, protective oversight, and supervision.
